Betahistine works in two ways. Firstly, the drug acts as a weak agonist of the histamine H1 receptors located on blood vessels in the inner ear. This effect on the receptors results in local vasodilation.
Secondly, it acts as an antagonist to histamine H3 receptors, which increases the levels of certain neurotransmitters released from the nerves in the ear. This also dilates the blood vessels, relieving pressure from the excess fluid buildup.
Additionally, It is hypothesized that betahistine inhibits the activity of vestibular nuclei (a part of the brain that deals with posture and balance) by increasing serotonin levels in the brainstem.
